Myeloproliferative neoplasms (polycythemia vera, essential thrombocythemia, primary myelofibrosis) are caused by an alteration of the hematopoietic stem cell in the bone marrow, namely the cell responsible for the production of red blood cells, white blood cells, platelets.

In myeloproliferative neoplasms, the uncontrolled proliferation of the hematopoietic stem cell causes an abnormal increase of these mature cells in the peripheral blood (mainly red blood cells in polycythemia vera, platelets in essential thrombocythemia, white blood cells in primary myelofibrosis).

MPN are chronic disorders, with a slow and progressive course. The increased number of red blood cells, white blood cells or platelets makes blood less fluid and can cause an increased risk of obstruction of veins or arteries (thrombosis). In a small percentage of patients, the disease can evolve to secondary acute myeloid leukemia with a poor prognosis.

Before 2005 the molecular basis of myeloproliferative neoplasms was unknown. Today we know that 90% of patients has a mutation in one of three genes: JAK2, CALR, MPL. In the remaining 10% of patients the molecular basis is unknown.

The discovery of mutations has stimulated the identification of targeted drugs such as the JAK inhibitors, able to selectively damage the mutated cells. Not all patients obtain a complete response with JAK inhibitors.

The project is aimed to characterize the impact of the three mutations on the clinical phenotype of the disease (Pavia), to identify the mutation/s responsible for the remaining 10% of patients with unknown pathogenesis (Vienna), to identify the molecular mechanisms responsible for resistance to JAK inhibitors (Boston).
