The project

“Hold in my hands a 3D printed model of the patient’s anatomy, look at it, analyze it from different angles, let me effectively plan and perform surgeries.” Prof Andrea Pietrabissa

Pancreas tumor, kidney cancer, spleen tumor, aortic aneurysm and dissection: these are just some pathologies where 3D printing has an effective impact on surgical operation planning procedure.

The physical model obtained through 3D printing shows all the anatomical features of the region of interest, highlights surgically relevant structures such as vascularization, position and extension of the tumor or lesions. The printed model is intuitive and carries a lot of information, which is very helpful for the surgeon to better understand anatomical relationships, to carefully plan surgical accesses and manoeuvres, thus to adopt an optimal intervention strategy.

In fact, at Policlinico San Matteo of Pavia, the Division of Chirurgia II headed by Prof. Andrea Pietrabissa exploits 3D printed models from patient’s CT images in daily routine: nowadays 50% of pancreatic surgery and 100% of spleen and kidney surgery benefits of 3D printed models. Also the vascular and cardiothoracic surgery Division at Policlinico San Donato are testing the use of our printed models in the decision-making process.

The first step for the creation of an anatomical model to support general and vascular surgery relies on preliminary medical image processing, much more complex than that used in other fields, such as dentistry, where 3D printer is already used in an extensive way. The surgeon provides the patient’s CT or MR images with indications on the anatomical regions of interest; the medical images are then processed to build an anatomical 3D virtual model, subsequently sent to the 3D printer. Such an approach is today explored by our research team in other several fields, as orthopedics and otolaryngology, even though the potential benefits of this technology could have a great impact on different medical areas.

The goal of this initiative is to extend the technology we have developed so far, at present limited to specific scientific collaborations, and make it available for every Medical Institute asking for our support. We want to create an actual integrated tool that helps the surgeon in the planning of procedures like pancreatic, renal, and splenic tumor resections, renal donation, splenectomy, treatment of aortic aneurysm and dissection.

So far, our equipment does not let us implement a real and effective integrated service: We need advanced instruments, enabling faster and cheaper anatomical model creation, and dedicated staff to interact with surgeons.

Goals

We set three different targets with increasing impact:

Initial target: 40.000€

Pilot service for the local community (dedicated 3D printer, printing materials, and part-time personnell)

Intermediate target: 70.000€

Regional service (dedicated 3D printer, printing materials, and full-time personnell)

Final target: 120.000€

National/international service (more performing dedicated 3D printer, printing materials, and full-time personnell)

Depending on the obtained funding, we plan to increase the user base, leading to the creation of a reference center.
